QR Code Lead Forms: Collect Contacts From a Scan

Put a form on a list-of-links or vCard page. The scan opens the page. The name they leave goes to your Contacts dashboard. Print once. Change the form later.

A QR code is good at getting a phone to a page. It is a worse tool for getting the person to tell you who they are. Most codes open a site, a PDF, or a social profile, and then the scanner is gone. A lead form sits on the page the scan already opened, so the same person can leave a name and a way to reach them without hunting for an email address on a poster. This is not its own QR type. There is no “form code” in the generator. You build a list-of-links page or a vCard, switch the contact form on, and print that code. The scanner sees the page first — your links, or the contact card they can save — and the form is on that same page. Submissions land in the Contacts dashboard, not in a mailbox you have to invent a workflow for. If you already have a site with its own form, point a URL code at that page and skip this. If you do not, this is the form. It only lives on those two hosted pages. A plain URL code, a PDF, a gallery, a Wi-Fi code, and the rest do not grow a form. Do not look for a CRM, a drip sequence, or an automation handoff here. The job is: scan, page, details, dashboard. You can switch the form on or off, or change the page around it, after the code is printed, because the pattern encodes a short link, not the page. The print stays. The form is the part you edit.

Six places a scan should also collect a name

A business card that works in both directions

A vCard lets someone save you. Add the form and the same scan can send their details back. That is the whole point of putting the form on the contact card they already opened — they came to take your number, and they can leave theirs without a second code, a second app, or a hunt for an email on the card. Useful at a conference table, where people will scan and keep walking. The printed card stays the same if you later take the form off, or change the page around it, because the pattern is a short link. The name they leave goes to your Contacts dashboard.

vCard QR code

A booth or table where people will not fill a clipboard

Put a list-of-links on the stand — Instagram, a brochure, a calendar — and put the form underneath. The scan opens the page. They can tap a link or leave a name. Nobody has to staff a clipboard or read handwriting later. The submissions land in the Contacts dashboard, tied to the code on that stand. If the booth moves to a new show, you change the page or take the form off. You do not reprint the stand. This is not a new QR type. It is the list-of-links page you were going to print anyway, with the form switched on.

List of links QR code

A property sign that should produce a viewing request

A vCard on the board gives the agent. Add the form for the viewing request on the same page the scan already opened. The person standing at the sign can save the agent and leave a name without hunting for an email on the board. When the house sells, edit the page. Do not reprint the board. The printed pattern encodes a short link, not the page, so the form is the part you change. Submissions go to the Contacts dashboard. A URL code, a PDF, or a gallery on that same board would not grow a form — only the vCard page does, of the two hosted pages the form lives on.

vCard QR code

A flyer that asks for a quote, not a website visit

Packaging, takeaway, and services flyers are good at getting a phone to a page and bad at getting a name. A list-of-links page can carry the offer, the hours, and the form underneath. The scan opens the page. The quote request is the form, not a hope that they will find a contact page later. If you already have a site with its own form, point a URL code at that page and skip this. If you do not, this is the form. Print the flyer once. Change the form later. The pattern does not freeze it.

List of links QR code

An event badge that should become a follow-up list

Put the form on a vCard. The badge scan opens the contact card they can save, and the form is on that same page. After the event, turn the form off. The print stays. You do not reprint the badges to close the list. The names they left are in the Contacts dashboard. There is no form code in the generator and no drip sequence here. The job is scan, page, details, dashboard — then take the form off when the campaign ends.

vCard QR code

A window or counter where the staff are busy

A salon, a clinic, a showroom: hours, WhatsApp, booking, and the form on a list-of-links page. The scan opens the page. People waiting at the glass can leave a name without pulling someone off the floor. The staff are busy; the clipboard will not get filled. Submissions land in the Contacts dashboard. You can switch the form on or off after the code is on the glass, because the printed pattern is a short link. A Wi-Fi code on the same counter does not grow a form. Neither does a plain URL. It only lives on the list-of-links page and the vCard.

How to add a lead form to a QR code

  1. 1

Start with a list of links or a vCard

There is no form code in the generator. Build a list-of-links page or a vCard. Those are the two hosted pages the form lives on. A plain URL code, a PDF, a gallery, a Wi-Fi code, and the rest do not grow a form.

  2. 2

    Switch the contact form on

The scanner fills it in the browser the camera opened. No app. They see the page first — your links, or the contact card they can save — and the form is on that same page.

  3. 3

Design the code and print it

PNG or SVG. The printed pattern is a short link; it does not freeze the form. You can switch the form on or off, or change the page around it, after the code is printed.

  4. 4

    Read submissions in Contacts

The name they leave goes to your Contacts dashboard, not a mailbox you have to invent a workflow for. Change the page or take the form off when the campaign ends. The print stays.

It is a form on a page, not a new QR type

There is no form code in the generator. You build a list-of-links page or a vCard, switch the contact form on, and print that code. The scanner sees the page first. The form sits on it.

Submissions land in Contacts

The name they leave goes to your Contacts dashboard. The job is scan, page, details, dashboard — not a mailbox you have to invent a workflow for.

The print does not lock the form

The pattern encodes a short link, not the page. You can switch the form on or off, or change the page around it, after the code is printed. The print stays. The form is the part you edit.

Frequently asked questions

What is a QR code lead form?+

A contact form that sits on the page a scan already opened. Most codes open a site, a PDF, or a social profile, and then the scanner is gone. With the form on the page, the same person can leave a name and a way to reach them without hunting for an email address on a poster. Submissions land in the Contacts dashboard.

Is this a separate QR code type?+

No. This is not its own QR type. There is no “form code” in the generator. You build a list-of-links page or a vCard, switch the contact form on, and print that code. The scanner sees the page first — your links, or the contact card they can save — and the form is on that same page.

Which QR types support the form?+

It only lives on those two hosted pages: a list-of-links page and a vCard. A plain URL code, a PDF, a gallery, a Wi-Fi code, and the rest do not grow a form. Do not look for it on those types.

Where do the submissions go?+

Submissions land in the Contacts dashboard, not in a mailbox you have to invent a workflow for. The job is scan, page, details, dashboard.

Can I add or remove the form after printing?+

Yes. You can switch the form on or off, or change the page around it, after the code is printed, because the pattern encodes a short link, not the page. The print stays. The form is the part you edit. Print once. Change the form later.

Do people need an app to fill it?+

No. The scanner fills it in the browser the camera opened. No app.

What if I already have a form on my website?+

Point a URL code at that page and skip this. If you do not, this is the form.

Add the form to a list of links or a vCard, then print the code once.
